In an attempt to increase feasiblity and safety of laparoscopic distal pancreatectomy with preservation of the splenic vessels, we apply a technique involving a medial-to-lateral dissection of the splenic vein from the pancreas, together with the use of modern hemostatic tools. This movie shows the case of a young patient having a symptomatic mucinous cystadenoma.

  • the patient is placed supine in the "french position"
  • the gastrocolic ligament is divided but the short gastric vessels are preserved
  • the pancreatic tail,including the splenic vein ,is separed from the retroperitoneum medially to the lesion
  • a vessel loop is placed to elevate the pancreas and for quick control of bleeding in the event a venous tear occurs
  • the splenic vein is gently freed from the pancreas in a medial-to-lateral dissection. Hemostasis of its pancreatic branches is performed using the 5 mm Ligasure
  • the proximal pancreas is divided using GIA stapler before completion of the dissection in the splenic hilum

In the reported case, the procedure took 4 hours and the patient was discharged on postoperative day 7. The described technique may be feasible and safe.
