The video demonstrates the use of an endoluminal device, the InScope Optical Dilator, that provides direct visualization when performing an esophageal dilation. It is a transparent flexible tube that can accommodate an endoscope up to 10mm in diameter and allows for continuous and gradual dilation with simultaneous visualization from within the dilator. We present two patients in which the Optical Dilator is used to dilate an esophageal stricture and mass. The first case is an 80 year old male with a history of oropharyngeal cancer who underwent a proximal esophagectomy with a jejunal interposition graft. He developed an anastomotic stricture over one year with complaints of dysphagia with solid and liquid food. Here the stricture was gradually dilated from 14 mm to 16 mm with complete resolution of his dysphagia. The second case is a 67 year old male with a recent diagnosis of distal esophageal adenocarcinoma with complaints of worsening dysphagia to solid and liquid food. While undergoing metastatic work up and evaluation for neoadjuvant therapy, he underwent an esophageal dilation and temporary esophageal stent placement using the Polyflex removable stent. Successful resolution of dysphagia after dilation with the Optical dilator and Polyflex esophageal stent placement.
