The video of our laparoscopic vagotomy and antrectomy demonstrates the entire procedure that includes: trocar placements, creation of the Roux limb and distal small bowel anastomosis, anterior and posterior vagotomies, antrectomy, and the Roux-en-Y gastrojejunostomy. The patient is a 62 year old female with recurrent pyloric ulcers for 3yrs. despite being on a PPI. She complained of intermittent nausea and mid-epigastric pain when eating. She denied any weight loss and used to smoke 2 ppd for over 40 years. Her upper GI series revealed a pyloric stricture for which she underwent multiple endoscopic dilatations. In addition, multiple EUS biopsies were negative for malignancy. Our video demonstrates the technical aspects of laparoscopic small bowel and gastric division and anastomosis with Endo GIA staplers and a laparoscopic hand-sewn closure of the gastrojejunostomy. Furthermore, we demonstrate our approach to the anterior and posterior vagotomies with highlights of the dissection, identification of the anatomic structures and resection. The final pathology showed histologic confirmation of both the anterior and posterior vagus nerves and mild chronic inflammation of the distal stomach. The post-operative course was uneventful and the patient tolerated a clear liquid diet by POD#5.
