Laparoscopic Resection of a Peripancreatic Cyst

Laparoscopic approaches to surgery of the pancreas have been previously reported. When feasible this approach offers patients the potential for decreased hospitalization and post-operative pain compared to an open approach. This video documents the enucleation of a peripancreatic cyst. The video illustrates the case of a 50 year old male who presented with acute pancreatitis. A computerized tomography scan of the abdomen revealed a cystic lesion abutting the mid body of the pancreas. Endoscopic ultrasonography was performed along with fine needle aspiration. This study characterized the cyst as a 4.5 by 2.5 cm mass containing abundant mucin and scant mucinous epithelium. Based on these findings the diagnosis of a mucinous cystic neoplasm of the pancreas was made. The patient was taken to surgery where a laparoscopic enucleation of the cyst was performed. The operation was completed with five trocars. A Harmonic scalpel was used to facilitate dissection. The lesion was discovered in the lesser sac of the abdomen attached to the superior aspect of the mid body of the pancreas. The lesion was dissected free from surrounding tissue and placed in a retrieval bag. Final pathology was consistent with a simple foregut cyst. The patient did well and was discharged home on the second postoperative day. This video illustrates the feasibility of the laparoscopic approach to cystic lesions in the region of the pancreas.
