Due to the spleen's important immunological function, preservation of the spleen is now seen as a top priority when surgery involves that organ, as is among others the case with mucinous cystadenomas of the pancreas, which should always be removed due to their tendency to become malignant. Minimally invasive surgery, though technically demanding, is a good option in such cases. This video presents the representative case of a young woman with a preoperative diagnosis of mucinous pancreatic cystadenoma, later confirmed by histology. The cystectomy with resection of the pancreatic tail was performed laparoscopically and the surgical technique including positioning of surgeon and equipment, placement of trocars and detailed course of the successful operation are shown in a manner that can be readily followed by a surgeon. It must be emphasized that great care must be taken when dealing with the vasculature of the spleen and pancreas, especially the splenic vein. Excessive devascularization of the pancreas is to be avoided as it could favor the development of necroses and fistulas. In this particular case, the cosmetic results and a CT at three months' follow-up were highly satisfactory.
