Cervicoscopic Esophageal Myotomy for a Pharyngoesophageal Motor Disorder

This video demonstrates the technique used to perform a cervical esophageal myotomy in a male patient with neurogenic pharyngoesophageal dysphagia. Barium swallow showed pharyngeal esophageal dysfunction and manometry was consistent with impaired sphincter opening and increased outflow resistance. The myotomy was done using four 3.5 mm operating trocars and CO2 gas insufflation of neck. The video demonstrates port placement, development of the operating space in the neck, identification of major vascular structures, recurrent laryngeal nerve and esophagus. The myotomy was performed using microscissors. To our knowledge this is the first attempt of a cervical myotomy performed endoscopically. The techniques used to determine the extent of the myotomy using intraoperative manometry is also demonstrated.
