Laparoscopic repair of paraesophageal hernia can be facilitated by anatomic excision of the hernia sac, which reduces hernia recurrence. This presentation discusses the necessary preoperative work up and major technical issues of sac excision, crural closure, degree of esophageal mobilization, and indication for an antireflux procedure in 75 years old anemic women.
