The laparoscopic technique for the dissection of large paraesophageal hernias differs significantly from the one for sliding hiatal hernias. In this presentation, the procedure is shown in a stepwise manner. Technical steps in reducing massive gastric herniations with organo-axial volvulus and the dissection of the sac are clearly demonstrated.
