One-fifth of patients with a paraesophageal hernia present with a potentially life-threatening complication. The presence of this abnormality, therefore, has been considered an absolute indication for operative therapy. The laparoscopic approach to this problem with reduction of the hernia, transection of the hernia sac, closure of the crural opening, and an associated Nissen fundoplication is presented in this video.
