This video shows the technical steps of laparoscopic fundoplication for large mixed or paraesophageal hernias in 8 patients. We perform: reduction of the herniated organ, crural dissection and suture, section of the short gastric vessels, and construction of a 360 wrap over a 56 Fr. calibrating Maloney. The wrap is sutured without including the esophageal wall with 3 interrupted stiches and is fixed cranially to the right crus and caudally with the Rossetti suture. Short and long-term results were satisfactory, thus we conclude that, even if more technically demanding than fundoplication for a simple GERD, laparoscopic fundoplication is feasible and safe also for large paraesophageal or mixed hernias.
