Curative laparoscopic colorectal resections for primary adenocarcinoma of the bowel are currently justified only if performed according to accepted principles of conventional oncologic colorectal surgery. This videotape uses a combination of surgical diagrams and intraoperative video to demonstrate a step-by-step approach to laparoscopic oncologic proctosigmoidectomy with precise adherence to these principles. Specifically, this video demonstrates: 1) use of laparoscopic hepatic ultrasonography for staging, 2) proximal ligation of the inferior mesenteric vessels with thorough regional lymphadenectomy, 3) wide en bloc resection of colon and mesentery, 4) mobilization of the splenic flexure using a novel retraction device, 5) protection of the peritoneal cavity and abdominal wall from contamination by placement of the specimen in an impermeable sac prior to removal, and 6) performance of a safe anastomosis utilizing a circular stapler.
