A 2 year old male with dysphagia and failure to thrive presented with an intramural cyst in the distal esophagus, luminal narrowing on esophagogram and an intact mucous membrane at esophagoscopy. The resection was transthoracic and extramucosal. Postoperatively, contrast study showed a wide lumen in an asymptomatic child.
