After endoscopic resection of pelvic endometriosis, nodular disease may present in the anterior rectal wall, requiring colonic resection. This can be very effectively accomplished through a transvaginal approach, exteriorizing the diseased segment of bowel through a posterior colpotomy. An hand-sewn anastomosis is then possible. This option can be a useful procedure in selected cases.
