Numerous cancers that occur within the true pelvis invade full-thickness through the involved structure and by gravity seed the pelvic cavity with malignant cells. Rectosigmoid cancer and ovarian malignancy are the most common examples. In this surgical procedure performed in a female, the complete pelvic peritoneum is removed en bloc so that further transcoelomic dissemination of cancer outside of the pelvis does not occur. Complete pelvic peritonectomy involves removal of the rectosigmoid colon, hysterectomy and bilateral salpingoophorectomy, plus total stripping of all peritoneum from the pelvic sidewalls. The colorectal or coloanal anastomosis is performed in the non-irradiated patient with a stapling device without a diverting colostomy.
