Esophageal achalasia is a poorly understood disease which causes severe symptoms and substantial morbidity in patients. Patients who have esophageal achalasia and who fail pneumaticdilatation often require surgical treatment. In the past, this surgical treatment would consist of an esophageal myotomy requiring a left thoracotomy or laparotomy. Recent developments with videoscopic surgery have alllowed us to do operations by minimally invasive techniques. Presented is an unusual case of achalasia with associated large esophageal diverticulum treated by thoracoscopic esophageal myotomy and resection of the esophageal diverticulum.
