Laparoscopic splenectomy is technically challenging because of the many vascular and peritoneal attachments. Splenomegaly is a contraindication. The splenic artery should be ligated in the lesser sac first or be embolized preoperatively. Autologous blood and a laparotomy tray should be available. Patients with successful procedures benefit with shorter hospital stay and less pain.
