A technique for trans-oral esophagectomy without thoracotomy for early cancer is presented. A guide inserted via the mouth is fixed to the lower end of the esophagus. The esophagus is stripped out through the mouth by pulling the guide upwards. The advantages of this operation are: less tissue dissection, minimal contamination, preservation of the vagus nerve, and, simplicity.
