Pheochromocytoma arises from the neural crest cells.They make, store, metabolize and release catecholamines; epinephrine, nor-epinephrine and dopamine. Most are adrenal in origin, unilateral and benign, approx 10% are bilateral, 10% are extra-adrenal,10% are in adults and up to 10% may be malignant.A young 20 years old female, presented to us with complaints of uncontrolled hypertension, episodes of headache, sweating and palpitations since five years. Patient had no symptoms of flushing, fever, nausea, vertigo or abdominal pains. Patient was taking anti hypertensives but her blood pressure remained uncontrolled. Her 24 hour Urinary VMA was at the upper normal limit i.e. 56 mg. Rest of biochemical profile was normal. CT scan abdomen showed a left sided retro-crural, para-aortic and juxta-adrenal mass of 4.6*2.8*6.7 cm. Patient was hence diagnosed to be suffering from extra adrenal para-aortic paraganglioma. Laparoscopic excision of the tumor was planned after controlling her blood pressure by alpha blockers followed by beta blockers.
