Laparoscopic approach to left adrenalectomy has proven value. The safest, most efficient, and most direct approach is through the splenic flexure colonic mesentery. Our video describes the direct approach through the mesentery, under the pancreatic tail, ligating the vein first. This approach has short operative times, accommodates large tumors, and particularly benefits pheochromocytoma.
