This video clearly describes a technique of laparoscopic left adrenalectomy in a patient with primary aldosteronism. Aldosterone levels were high and renin levels were low. CT scan showed a 2.2- cm mass of the left adrenal gland. The patient underwent a two hour laparoscopic left adrenalectomy and was discharged on postoperative day two. At the time of discharge, the patient's blood pressure had been normal without antihypertensive drugs. The adrenal gland was approached through the root of the transverse mesocolon to the left of the inferior mesenteric vein, exposing the adrenal gland. This technique does not require mobilization of colon or spleen. For patients with benign disease, laparoscopic adrenalectomy is a useful, safe alternative that allows the patient return to normal activity within a shorter time.
