Various pouch configurations are described in performing a restorative proctocolectomy. The S pouch is constructed from three limbs of terminal ileum with the distal limb used to construct an end-to-end anastomosis. The advantage of this pouch design is that there may be less tension at the IAA than with the J pouch. However, it is more time consuming to construct and is generally not amenable to construction using staplers.
