The anterior sagittal pull-through for imperforate anus is a simplification of the Mollard perineal approach for high imperforate anus. It provides a safe dissection parallel of the urethra, preserves the puborectalis and the internal sphincter around the fistula, and creates a short anal canal surrounded by striated muscle and lined by anoderm and transitional epithelium.
