The primary principle to be maintained in this technique is to ensure wide enough dissection both distally and particularly circumferentially in the true pelvis so as to minimize the chances of local recurrence. With the improvements in stapling equipment, this has become feasible for increasingly more distal tumors, thus preserving sphincter function. With care the sexual nerves also can be protected, thereby not only allowing a proper cancer procedure but improvement in quality of life.
