Traditionally, buccal mucosal grafts have been used in urethral reconstruction; however rectal mucosal grafts have emerged as an option due to less postoperative pain and yielding larger graft sizes. We present a case of a 25 year old male, who sustained a five centimeter urethral defect due to a gunshot wound to his perineum, and underwent a rectal mucosal graft harvest for a staged urethroplasty. For our operative approach, we employed transanal minimally invasive surgery (TAMIS) using a laparoscopic multi-port platform device. The dimensions of the rectal mucosa graft harvested were 3cm wide by 12 cm long. The graft was then prepared by the urology team and used for urethral reconstruction as would a buccal mucosal graft. The patient had no intraoperative or postoperative complications. We conclude that rectal mucosal graft harvest via TAMIS approach is a safe and effective method for urethral reconstruction.
