A patient with diarrhea and weight loss was diagnosed as cancer of the right colon with duodeno-colonic fistulae. Big size adenocarcinoma with extension to duodenum and pancreas with tumoral fistulae was demonstrated. Clinical records are included. The patient was treated by right colectomy plus duodeno-cephalic pancreatectomy (on-block resection). The movie shows mobilization of the right colon, pancreas and duodenum. Dissection of the Riolano's Arcade, superior mesenteric branches, section of the ileum and transverse colon. Dissection of portal vein, celiac axis and splenic vessels. Section of the pancreas and gastro-duodenal artery. Section of the common duct and mobilization of the gallbladder. Section of the duodenum below the pylorous. Reconstruction was done by Wirsung-jejunostomy. Finally ileo-transversostomy. The movie includes follow-up and X-Ray examinations.
