This 19-month-old infant with a cloaca and prune belly syndrome had previously undergone colostomy, vesicostomy, and right nephrectomy. Cloacal repair included pullthrough of two high vaginas using a bowel segment to lengthen them, abdominal perineal rectal pullthrough and vesicostomy closure. Both posterior sagittal and abdominal approaches were used.
