Cloaca denotes confluence of the bladder, vagina, and rectum into a urogential sinus. This case with two vaginas was operated upon both posterior midline and anterior abdominoperineal approaches. All three organ systems were separated and reconstructed. It required extending the high vaginas to the perineum with bowel. Anatomic and functional results were satisfactory.
