This video illustrates the technique of ileal J-pouch anal anastomosis fashioned with staplers. Some technical considerations are: 1. Preservation of the ileocolic artery, though not indispensable in fashioning the J-pouch, allows more length and a better vascularization of the pouch. 2. Rectal mucosectomy should start at the dentate line so as to leave no diseased mucosa at all, in order to avoid possible bleeding. This video also illustrates the postoperative care until ileostomy closure and, in particular, the self-training of continence and evacuation.
