Title: Laparoscopic excision of symptomatic urachal remnant cyst Purpose: To describe the laparoscopic management of symptomatic urachal remnant cyst in adulthood Background: Patent urachal cyst is the incomplete regression of the fetal urachus. Most urachal abnormalities present in childhood. Presentation of urachal abnormalities in adulthood is rare with recurrent infection being the most common presentation. Delay in diagnosis is often the case unless the diagnosis is suspected and confirmed with imaging Methods: A 37-year old male presented with recurrent umbilical pain and swelling for 8 months. Occasional dysuria was noted along with drainage of blood tinged pus from the umbilicus. He was treated with antibiotics for 15 and 20 days on 2 separate occasions. CT scan was read normal and patient was referred to us. However, upon our personal review of the CT scan, we noted a patent urachus connecting the umbilicus to the dome of the bladder Interventions: Laparoscopic excision of remnant urachal cyst via 3 trocar technique along with closure of the bladder Outcomes: Immediate improvement in the symptoms. No recurrence noted at 4-month follow-up. Conclusions: Remnant urachal cyst in adulthood is rare but such patients may be seen by the colorectal surgeon. Suspicion is needed for diagnosis and it should be confirmed with imaging. Laparoscopic excision of the cyst is technically feasible with good outcome.
