Valentino's syndrome is used to describe patients presenting with right lower quadrant pain very similar to acute appendicitis, however is actually caused by perforated gastric/duodenal ulcer having the succus flow down and collect in the right parabolic gutter. Duodenal diverticulum, which is the most common location for small bowel diverticulum, has a reported 7% incidence rate and mostly near the ampulla. Majority of them are asymptomatic, and rarely have complications. We are presenting a 26-year-old male with Valentino's syndrome caused by a perforated duodenal diverticulum, which is repaired laparoscopically. To our knowledge, it will be the first case report and addended with laparoscopic repair of Valentino's syndrome caused by perforated duodenal diverticulum.
