Large splenic artery aneurysm is a potentially life-threatening condition because of the risk of aneurysm rupture. Splenic hemangioma is a rare disease and also very unusual condition when found with a splenic arterial aneurysm. The challenges, in this case, are how to localize the aneurysm as well as deal with a vascular control. We decided to operate by robotic surgery, which allows for fine vessel dissection and precise movement through the endowrist. We presented a 69-year-old woman who had a history of asymptomatic chronic thrombocytopenia for 4 years. Abdominal exam and ultrasonography revealed splenomegaly with splenic mass. Moreover, abdominal computed tomography showed an 8 cm, well defined, hypodensity splenic mass and a 5 cm thrombosed splenic arterial aneurysm incidentally which located at an atypical location. Robotic splenectomy and aneurysmectomy was planned for this patient. The patient was placed in supine position. The robotic cart was docked from the patient's head. Medial to lateral approach was performed and both splenic vessels were individually controlled. The splenic aneurysm was en-bloc resected with the spleen, Finally, splenectomy was completely done. The patient recovered well and was discharged uneventfully on postoperative day 3. The video demonstrates the technical feasibility of robotic splenectomy with aneurysmectomy for splenic tumor with large splenic arterial aneurysm.
