Treatment of Iatrogenic Lesion of the Common Duct with Section of the Right Hepatic Artery

A patient with cholelithiasis was treated by laparoscopic cholecystectomy, suffering choledochous resection and right hepatic artery section. The patient was sent to us with a local drainage and percutaneous transhepatic external pig-tail, with severe sepsis. After haemodynamic control, treatment of the sepsis and TPN the patient was surgically treated. Relaparotomy was done, removal of the hepatic necrotic tissue and dissection of both hepatic ducts. Th left hepatic duct was enlarged by means of longitudinal section. The right duct increased the diameter by section of one centimeter length laterally. A Roux-en-Y 60 centimeters length was prepared and mobilized to the hilium of the liver behind the duodenum through a incision on the transverse mesocolon. Bilateral anastomosis was done between left hepatic duct and right duct with the jejunum (Double Biliary Diversion) using two separated sections on the jejunum. Only one layer interrupted stitches were used (Monofilament 5/0, absorbable material). The presentation finished with the follow-up of the patient.
