Cervicothoracic thymic cysts are among the rarest of congenital neck masses. Thorascopic surgery may enable the treatment of the mediastinal component of the cyst, decreasing the surgical morbidity. We present the case of a large cervicothoracic thymic cyst successfully treated with the combination of thorascopic resection and cervical dissection.
