The patient is a 2 1/2 year old male, previously asymptomatic until he presented to an outside ER with shortness of breath. The X-ray demonstrated a large left diaphragmatic hernia. He was transferred from the ER for definitive care. Because the child was clinically stable, it was elected to repair the defect via a thoracoscopic approach.
