Surgery for giant bullous disease has been described for patients with significant pulmonary compromise. We describe the management of giant bullous disease using a pure thoracoscopic approach. In light of donor scarcity, this may be a useful alternative in lieu of lung transplantation for patients with severe COPD and unilateral giant bullae.
