Persistent congenital tracheobronchial malacia is a difficult entity to treat. A unique surgical approach was undertaken for stabilization of the trachea and mainstem bronchi in an 18-month-old, who was completely ventilator dependent with total muscle paralysis. Mesh wrap and autologous rib stenting prevented collapse of the upper airway allowing for discontinuation of ventilatory support and eventual discharge home.
