A 38 year old female, who had received a Beall valve replant 21 years prior to admission, presented to the operating room in extremis. The patient had no blood pressure, urine or pulses. She underwent bypass for resuscitation. The Beall valve was totally and completely obstructed with an acute and chronic clot. We used the "beehive balloon retrograde catheter" for retrograde warm/cold/warm cardioplegia which was designed to be in a totally occlusive balloon, espeically in a patient with an enlarged coronary sinus often associated with valvular disease. The patient left the hospital without complications on the tenth postoperative day.
