A 68 year old white male developed acute myocardial infarction and Class IV angina pectoris after gallbladder surgery for jaundice and cholelithiasis. Significant past history revealed laryngectomy, radiation therapy and permanent tracheostomy flush with the sternal notch. Cardiac exposure was obtained by modified sternotomy with a "T" shape incision. Details of the technique used are shown.
