The surgical approach to giant mediastinal germ cell tumor of the right chest is demonstrated. MRI, CT scan, and angiogram demonstrated complete obstruction of the right main stem bronchus. This resection required removal of the tumor from the chest, preferably without bypass, due to the vascular nature of the adhesions following chemotherapy. Following the rather extensive dissection of an extremely unique tumor weighing more than six kg and measuring 22 x16 cm, the right lung was allowed to re-expand dramatically.
