Vagotomy and antrectomy is the treatment of choice for gastric outlet obstruction due to complicated chronic duodenal ulcer. Not only does it reduce acid secretion, but it removes a distended vagotomized antrum, site of gasrin production. This procedure can be done safely laparoscopically. Two Billroth techniques are demonstrated.
