We present the case of a 61-year-old male patient with a diagnosis of morbid obesity. His past medical history included a vertical banded gastroplasty in 1998.

Seven years after surgery, the patient presented with a 10-month history of weight regain.

His initial assessment demonstrated a fistula between the gastric pouch and the gastric remnant. The patient was then taken to the operating room for a revision of his previous bariatric procedure.

This video demonstrates the preoperative findings of a gastro-gastric fistula in a patient with a previous bariatric procedure. It also demonstrates the laparoscopic surgical technique for the conversion of a vertical banded gastroplasty to a Roux-en-Y gastric bypass, as well as the postoperative appearance of the anatomic reconstruction.
