Pouch enlargement (PE) and band slippage (BS) are frequent complications after Laparoscopic adjustable gastric banding (LAGB). The presence of a fibrotic ring underneath the band is an intraoperative finding frequently associated to PE and BS. 37 year old female, complaining of vomiting for 4 days. Similar episode 3 months ago, due to PE. Fluid was withdrawn from the band at that time. No adjustment since then. PSH: LAGB in 2002. Gastrografin swallow: PE and no passage of contrast. 55 year old female complaining of dysphagia for 2 months. PE 6 months ago. Fluid was withdrawn at that time. No adjustment since then. PSH: LAGB in 2001. Gastrografin swallow: no passage of contrast, band in good. After failure of medical treatment, surgical approach was decided. A fibrotic ring underneath the band was found intraoperatevely. Band was removed in the 1st patient and the scar ring was transected in the 2nd patient. Operative time was 45 and 30 minutes while hospital stay 48 and 72 hs respectively. The presence of a fibrotic ring LAGB should be considered whenever medical treatment is unsuccessful during management of PE or dysphagia in the absence of PE.
