We present a case of a laparoscopic revision of a nonadjustable gastric band to a Roux-en-Y gastric bypass. Our patient underwent this procedure seven years earlier and failed to lose weight and had worsening reflux. He desired a conversion to a gastric bypass. Six ports are placed and the dissection is started by isolating and removing the nonadjustable gastric band. Once this is done, a gastric pouch is created by dividing the stomach above the area of the gastric band. A laparoscopic Roux-en-Y gastric bypass is then created in the normal fashion. There were no post operative complications and the patient is doing well at 3 months follow up. Revisional laparoscopic bariatric surgery is safe and effective.
