This video presents a laparoscopic highly selective vagotomy in a 71 year old female, H. pylo ri negative, with recurrent peptic ulcers despite H2blockers and omeprazole administration. Use of the harmonic scalpel for division of the vagal branches along the lesser curvature of the stomach decreases operative time and prevents conduction injury to adjacent structures especially the vagus nerves, while providing excellent hemostasis.
