The common denominator in bile duct injuries is the failure to identify the structures of the triangle of Calot. Based on experience with eighteen patients referred within an eighteen month period, the predisposing factors, forms of presentation, and principles for prevention of these injuries are outlined. Basic concepts for reconstruction are presented by showing clinical cases.
