Following harvesting of a whole donor organ, the liver is split along the anatomical border to provide a right lobe and left lateral segment for orthotopic transplantation. The right lobe contains the major vessels, i.e., common hepatic artery, portal vein, right hepatic duct and the entire vena cava. The left lateral lobe is dissected along the falciform ligament and its vascular pedicles are reconstructed by vascular allografts. Venous outflow is provided through the left lateral vein and anastomosed either to the recipient IVC, or by means of a venous interposition graft of the median left vein. Orthotic replacement of the diseased liver is then demonstrated.
