Proximal biliary tumors frequently occlude both the right and left hepatic ducts, and often extend up into the liver parenchyma of the right and/or left lobes. The operative procedure performed on this patient includes resection of the extrahepatic biliary tree, including the bifurcation, and left hepatic lobectomy to remove the tumor that has extended up into the hepatic parenchyma, and had metastasized intraductally to the parenchyma of the left lobe. Reconstruction of the biliary tree using biliary silastic transhepatic stents and a Roux-en-Y loop is demonstrated.
