A three-year old male child with a huge hepatoblastoma of the left liver invading the portal vein was initially treated with neoadjuvant chemotherapy. The tumor decreased significantly in size, but had occluded the left portal vein and was protruding into the portal bifurcation. Extended left hepatic lobectomy with repair of the portal vein was carried out.
