Resection of the liver central portion is not frequently performed. For that reason, have been included two cases of mesohepatectomy; the first one due to focal nodular hyperplasia and the second one due to hepatocellular carcinoma. The video shows the special care we had in the dissection, clear and concise, of the vasculo-biliary elements of the hepatic hilium and at the intraparenchymatous level. In the second patient, the resection of the hepatocarcinoma forced to derivation of the three segmentary biliary ducts and a Y-Roux loop.
