This video presents a case demonstrating the structures in-situ in a patient with hilar cholangiocarcinoma. Excision of the common bile duct and a supra-duodenal lymphadenectomy with an extended right hepatic lobectomy is performed. A roux-en-y hepatico-jejunostomy is used for the left hepatic duct. Control of the right hepatic artery and division of left hepatic duct is illustrated and the Pringle maneuver is used for control of the portal vein and left hepatic artery.
